This document corrects technical errors in the final rule with comment period that appeared in the November 25, 2025 issue of the Federal Register titled "Medicare Program: Hospital Outpatient Prospective Payment and Ambulatory Surgical Center Payment Systems; Quality Reporting Programs; Overall Hospital Quality Star Rating; Hospital Price Transparency; and Notice of Closure of a Teaching Hospital and Opportunity To Apply for Available Slots" (hereinafter referred to as the "CY 2026 OPPS/ASC final rule with comment period"). The effective date of the CY 2026 OPPS/ASC final rule with comment period was January 1, 2026.
This regulation establishes an exemption from the requirement of a tolerance for residues of rice bran wax (CAS Reg. No. 8016-60-2) when used as an inert ingredient (lubricant) on growing crops and raw agricultural commodities pre- and post-harvest, when applied to animals, and in antimicrobial formulations applied to food-contact surfaces in public eating places, dairy-processing equipment, and food- processing equipment and utensils. Micro Powders, Inc., submitted a petition to EPA under the Federal Food, Drug, and Cosmetic Act (FFDCA) requesting establishment of an exemption from the requirement of a tolerance. This regulation eliminates the need to establish a maximum permissible level for residues of rice bran wax when used in accordance with the terms of those exemptions.
This rule revises the provisions of the Agriculture Risk Coverage (ARC), Price Loss Coverage (PLC), and Dairy Margin Coverage (DMC) programs to conform with provisions of the One Big Beautiful Bill Act (OBBBA). OBBBA authorized modifications to the 2025 crop year ARC and PLC programs and the continuation of the ARC and PLC programs for the 2026 through 2031 crop years. The modified provisions are related to the reference prices, the effective reference prices, base acres, program elections, and payment provisions. OBBBA also authorized DMC for calendar years 2026 through 2031, providing participating dairy operations with the ability to establish a new production history. In addition, the Tier 1 coverage level was increased by 1 million pounds of milk to a 6-million-pound limit and eligibility for multi-year (lock-in) contracts was maintained until December 30, 2031. The Farm Service Agency (FSA) is also making minor administrative changes and updates to the ARC, PLC, and DMC regulations and the regulations that apply to multiple FSA programs.
The Postal Service is revising Mailing Standards of the United States Postal Service, International Mail Manual (IMM[supreg]), and Notice 123, Price List, to reflect prices, product features, and classification changes to Competitive Services and other changes, as established by the Governors of the Postal Service.
The OCC, the Board, and the Bureau are finalizing amendments to the official interpretations for their regulations that implement section 129H of the Truth in Lending Act (TILA). Section 129H of TILA establishes special appraisal requirements for "higher-risk mortgages," termed "higher-priced mortgage loans" or "HPMLs" in the agencies' regulations. A December 2013 rulemaking exempted transactions of $25,000 or less and required that this loan amount be adjusted annually based on any annual percentage increase in the Consumer Price Index for Urban Wage Earners and Clerical Workers (CPI- W). Based on the CPI-W in effect as of June 1, 2025, the exemption threshold will increase from $33,500 to $34,200, effective January 1, 2026.
This final rule with comment period revises the Medicare Hospital Outpatient Prospective Payment System (OPPS) and the Medicare Ambulatory Surgical Center (ASC) payment system for calendar year 2026 based on our continuing experience with these systems. We also describe the changes to the amounts and factors used to determine the payment rates for Medicare services paid under the OPPS and those paid under the ASC payment systems. In addition, this final rule with comment period announces the closure of a teaching hospital and the opportunity to apply for available slots, and updates and refines the requirements for the Hospital Outpatient Quality Reporting Program, Rural Emergency Hospital Quality Reporting Program, Ambulatory Surgical Center Quality Reporting Program, Overall Hospital Quality Star Rating, and hospitals to make public their standard charge information and enforcement of hospital price transparency, as well as summarizes comments received in response to a request for information on measure concepts regarding Well-Being and Nutrition for consideration in future years for the OQR, REHQR, and ASCQR programs.
The Federal Energy Regulatory Commission is withdrawing a notice of proposed rulemaking, which proposed to amend its regulations to codify the Safe Harbor Policy established in the Commission's Policy Statement on Natural Gas and Electric Price Indices. The Commission is also terminating this rulemaking proceeding.
In this document, the Federal Communications Commission (Commission) seeks comment on its proposed rules to eliminate rate regulation and tariffing obligations for business data services provided by incumbent local exchange carriers in light of technological and marketplace changes and recent Executive Orders and Commission initiatives. The Commission alternatively seeks comment on updates to its regulatory framework and competitive market tests to better align with current market conditions based on current data.
In this document, the Commission temporarily waives Sec. Sec. 61.50(j)(2) and 69.803(c) of the Commission's rules until the Commission directs the Wireline Competition Bureau to update the competitive market tests for incumbent local exchange carriers' business data services.

This rule also contains requests for information on measure concepts regarding Well-Being and Nutrition for consideration in future years for all three programs (OQR, REHQR, and ASCQR; expanding the method to control for unnecessary increases in the volume of covered OPD services to on- campus clinic visits; software as a service; and adjusting payment under the OPPS for services predominately performed in the ambulatory surgical center or physician office settings.
The Coast Guard is establishing a temporary safety zone on the waters of the Monongahela River from mile marker 68 to mile marker 68.5 in Rices Landing, PA. This action is necessary to provide for the safety of life on these navigable waters from potential hazards during the Rices Landing Fireworks Display, for the activities planned on July 4, 2025. This proposed rulemaking would prohibit persons and vessels from being in the half-mile regulated area unless authorized by the Captain of the Port Pittsburgh or a designated representative.
On April 9, 2025, the Postal Service published notice of mailing services price adjustments with the Postal Regulatory Commission (PRC). The PRC concluded that the price adjustments contained in the Postal Service's notification may go into effect on July 13, 2025. The Postal Service will revise Notice 123, Price List, to reflect the new mailing services prices.
This direct final rule amends regulations that govern tobacco price support services established under the Agricultural Act of 1949. The Fair and Equitable Tobacco Reform Act of 2004 eliminated the price support and quota system for tobacco produced in the United States. In alignment with Executive Order 14192, AMS is removing and, where appropriate, amending regulations that have expired authorizing statutes and govern non-operational programs.
This document amends the Table of TV Allotments (table) of the Federal Communications Commission's (Commission) rules by substituting channel 15 for channel 11 at Price, Utah in response to a Petition for Rulemaking filed by Gray Television Licensee, LLC (Gray), the permittee of new full power television station KCBU, channel 11, Price, Utah. The staff engineering analysis finds that the proposal is in compliance with the Commission's principal community coverage and technical requirements. The substitution of channel 15 for channel 11 in the table will not result in any loss of existing service and it will also allow Gray to avoid the known viewer receptions issues on its currently authorized VHF channel.
The Postal Service proposes to revise, Notice 123, Price List, to reflect changes coincident with the recently announced mailing services price adjustments.
The Video Division, Media Bureau (Bureau), has before it a petition for rulemaking filed July 15, 2024, as amended on January 6, 2025, by Gray Television Licensee LLC (Petitioner), the permittee of unbuilt full power television station KCBU, channel 11, Price, Utah (Station or KCBU). The Petitioner requests the substitution of channel 15 for channel 11 at Price, Utah (Price), in the Table of TV Allotments.
